Dr. Thomas Seyfried and Dr. Dominic D’Agostino are prominent voices advocating for a paradigm shift in understanding cancer—not as primarily a genetic disease, but as a metabolic disorder rooted in mitochondrial dysfunction. Their collaborative work builds on Otto Warburg’s early 20th-century observation that cancer cells favor fermentation (glycolysis) for energy production, even in the presence of oxygen—a phenomenon known as the Warburg effect.
Seyfried posits that genetic mutations commonly associated with cancer are often secondary consequences of impaired cellular metabolism rather than the root cause. He argues that damage to mitochondria, the cell’s energy-producing organelles, drives malignant transformation by forcing cells into a fermentative metabolic state dependent on glucose and glutamine.
Dr. D’Agostino supports this view, emphasizing how metabolic therapies—such as nutritional ketosis, calorie restriction, and hyperbaric oxygen—can selectively target cancer cells while supporting normal cell function. Together, they have developed the Press/Pulse therapeutic strategy, which involves suppressing (pressing) tumor fuels like glucose and glutamine while pulsing other stressors (e.g., oxygen, fasting-mimicking diets) to enhance cancer cell vulnerability.
The Press/Pulse Strategy Explained
The Press/Pulse model is a metabolic framework designed to manage cancer by exploiting the unique vulnerabilities of tumor cells:
- Press: Chronically restrict fermentable fuels—glucose (via ketogenic diet) and glutamine (via dietary/pharmacological means)—to weaken cancer cells.
- Pulse: Intermittently apply non-toxic stressors such as hyperbaric oxygen therapy, fasting, or low-dose chemotherapy to selectively kill metabolically compromised cancer cells while sparing healthy ones.
This approach aims to shift the body into a state of nutritional ketosis, where normal cells thrive on ketone bodies, but cancer cells—unable to efficiently utilize ketones due to mitochondrial defects—starve and become more susceptible to oxidative stress.
The strategy is especially highlighted in aggressive cancers like glioblastoma (GBM), where conventional treatments offer limited success. Seyfried and D’Agostino argue that metabolic therapy could significantly extend survival and improve quality of life.
Clinical Applications and Challenges
Despite promising anecdotal evidence and preclinical research, metabolic therapy is not yet part of standard oncology care. One major barrier is the lack of registered dietitians trained in ketogenic therapies within cancer centers—outside of pediatric epilepsy programs.
Seyfried and D’Agostino emphasize that cancer patients need personalized metabolic coaching, biomarker monitoring, and multidisciplinary support to implement these protocols safely. They caution against unsupervised extreme diets, which could lead to malnutrition or other harms.
They also highlight that acidic tumor microenvironments, fueled by lactic acid and succinic acid from fermentation, impair conventional therapies like radiation and immunotherapy. By normalizing metabolism, metabolic therapy may enhance the efficacy of these treatments at lower, less toxic doses.
Future of Metabolic Oncology
Seyfried and D’Agostino envision a future where metabolic oncology becomes an integral part of cancer care. They call for more systematic reviews, randomized controlled trials, and education within the medical community to validate and scale these approaches.
While they avoid using the word “cure,” they define success as dramatically extending high-quality survival—turning lethal cancers into manageable chronic conditions. With growing interest and patient demand, they believe metabolic therapy will eventually become standard practice.
